Easily Access Your Imaging Reports and Medical Records

You may obtain your personal imaging reports and/or images the following ways:

Open, fill out and submit the secure Release of Medical Records form

Element Medical Imaging will securely send your radiology reports and images to your healthcare provider. You can conveniently access your radiology reports through the Patient Dashboard. If you need a physical copy of your images, there is a $10 fee.

Our Patient Dashboard is a secure, HIPAA-compliant platform providing you with real-time access to your radiology scans and reports. Additionally, you can conveniently schedule and manage your imaging appointments through this site. To get started, click here to create an account or log in if you are already a user.

Allow one business day for all requests.
You will be required to show a picture id when picking up your records and may be asked to verify other information.

If you are a third party
